PHYSICAL EXAMS
PHYSICALS FOR THE UPCOMING SCHOOL YEAR ARE DUE:
AUGUST 1
ALL Band and Colorguard students must have a current physical exam on file in order to participate. This must be done EVERY YEAR. It is a requirement of both the Pasadena Independent School District and UIL.
Students will not be permitted to participate in any marching band or colorguard rehearsal until completed.
NOTE: There are two steps to completing the process.
Students will not be cleared to participate until BOTH steps are completed:
Step 1: Complete Physical Exam
This may be done on-campus or at a doctor of your choosing.
Students MUST use the official PISD Physical Exam Form - It is the ONLY physical exam form that will be accepted.

or you may request a hard copy from your student's campus.
Completed physical forms should be turned in to any of the band directors.
If you completed your physical on campus, we will obtain a copy from the athletic department.
Your exam form must be SIGNED and STAMPED by your physician or it will not be accepted.
The same physical exam is good for BOTH Band and Athletics!!!

Step 2: Additional Documents
All additional documents will be completed online via the Rank One Parent Portal.
RETURNING PARENT/STUDENT
If you already have a Rank One account for your student, please log in and complete all required documents for the new school year.

NEW PARENT/STUDENT
If you are new to Rank One, you may follow the instructions below on how to set up your account.
Click the Button Above to access the Rank One Parent Portal, then follow the steps of the Registration Process listed below:
REGISTRATION PROCESS
1. To find your program, Click the Button Above to access the Rank One Parent Portal, and select the PARENTS CLICK HERE button (Red Button, Top Left Corner).
2. Next, click on the ONLINE FORMS button.

Select Pasadena ISD
3. You should be on the Welcome to the Parent Portal page. Select the Proceed to Online Forms button.
4. Create a New Account. First Name, Last Name, Email.
You will receive an email to Confirm your account.
5. Search for your student using your student's last name and student ID number.
6. You can Find Another Student or Start Forms.
Select the View button to get to the forms.
7. Select each Electronic Document and complete it.
Both parent and student should sign the forms.
8. You can select either a PDF or a Picture for the physical upload. Examples are given of what forms need to be uploaded.
All physicals must be uploaded.
Using your phone for the physical upload is easier since you just have to take a picture and upload but you can use a computer or phone.
9. Complete the six (6) electronic forms by selecting the forms and filling in all the required boxes and signatures.
10. When each form is completed, it will mark the form Approved.
You can come back to your forms using your email and password.
